Insurance: We accept most major insurance plans. Please contact our office to get more information.
Copays: Copays are due at time of service and will be collected while checking into your appointment. We accept cash, check, Visa, Mastercard, and Discover.
Office Visits : When preparing for your visit, we ask that you please bring your insurance card, copay and the medication bottles (including over the counter) with you to EVERY appointment. This is to better service you.
No Shows: We understand everyone has a busy life and schedule. Our ability to provide quality healthcare becomes challenging when patients are late or no show for appointments. We ask all of our patients to please call at least 24 hours in advance if they are unable to make their appointment. Please note there will be a $35 no show fee if no notification is received. After the patients third no show, dismissal from the practice may occur.
Refills: When requesting all refills, including any controlled substance, please allow the office staff at least 72 hours to fulfill your request.
